To reduce paper passing, we offer electronic check-in via our website. Simply fill out your e-forms here Patient Form submit them with the COVID-19 questionnaire to ensure you will be healthy and ready for your appointment.
To limit the amount of people in the office at one time, we offer car side check-in for patients by phone. Simply give us a call at 845-279-3720 to let us know you've arrived and we will let you know when we're ready for your appointment. We ask that you come in alone to maintain social distancing protocols.
*Patients under 19 permitted to have one parent/guardian with them. If you require assistance getting to the office, please ask your companion to remain in the vehicle during your visit.
Upon entering reception room, an assistant will take your temperature and ask you some screening questions regarding your current health. If all are satisfactory, you will proceed to your appointment. We ask that all patients enter with a mask on, if not, one will be provided.
PDA has always prioritized the use of PPE as a standard in protection of dental treatment. We've now added more PPE for the entire team to ensure the protection of both our staff and our patients. We may look a little funny, but we promise we are all smiles underneath all the protection!
We understand that you've waited a long time for your appointment due to Covid-19 shutdowns and we are eager to serve you once again. We are working hard to reschedule hundreds of patients based on dental emergencies and in the order the appointments were originally cancelled.
